Basic Policy on the Protection of Personal Information

1.Basic Policy

In recognition of the importance of personal information, Spatel Co., Ltd. ("Spatel" hereinafter) complies with the Act on the Protection of Personal Information ("Act" hereinafter) and the Guidance for the Appropriate Handling of Personal Information by Medical or Care-related Service Providers("Guidance" hereinafter) and other related laws in order to ensure the protection of personal information.

2. Specific Efforts

Spatel handles personal information in the following manner.

  • (1) Spatel will comply with related laws and regulations, including the Act and the Guidance.
  • (2) Spatel will formulate rules regarding the handling of personal information, appoint a person in charge of handling personal information, and all employees will comply with it.
  • (3) Spatel will take safety management measures for the proper storage of personal information and strive to prevent leakage, loss and damage.
  • (4) Spatel will regularly check that personal information is handled properly, and if any problems are found, will improve them.
  • (5) When acquiring personal information, Spatel will specify the purpose of use in advance and will not use it for any other purpose. However, this does not apply when the consent of the individual is obtained, when required by law, or when anonymization is performed so that an individual cannot be identified.
  • (6) When outsourcing operations, Spatel will ask the outsourcee to handle the basic policies with a thorough understanding, and will endeavor to take necessary supervision and improvement measures.
  • (7) Spatel will establish a consultation system regarding the handling of personal information and respond appropriately and promptly.

3. Handling of Anonymously Processed Information

  • (1) Spatel creates the following anonymously processed information when dispensing information. When creating anonymously processed information, Spatel takes the necessary measures to prevent identification of specific individuals or restoration of the personal information used in such creation.
    -Name of patient, birthday of patient, insurance number of patient, name of the doctor, prescription day and dispensing day.
    The following information is anonymously processed information.
    -Sex, birth year, the history of dispensing
  • (2) The anonymously processed information created by Spatel is provided to a third party for the purpose of data analysis such as management improvement by secured method.

4. Consultation System

Spatel will respond appropriately and promptly to any requests made by an individual regarding the following matters.

  • (1) When it is difficult to agree with the purpose of use of personal information.
  • (2) Disclosure, correction, suspension of use, etc. of personal information (except when prohibited by law).
  • (3) When personal information is leaked, lost or damaged, or when the possibility is suspected.
  • (4) If you have any questions or concerns regarding the handling of personal information.
